I understand that if I am employed by Old Forge Properties that my employment with Old Forge Properties will be for an indefinite period and may be terminated by the employer at any time with or without notice and with or without cause, except as limited by a written contract.

I also understand that electronic surveillance equipment will be present in my work area and that I may be video taped as a result of the system.

In the event of employment, Old Forge Properties and its authorized agents shall be entitled, without further consent, to copyrights, sell, or use in any manner, any picture, electronic photography, or recording of my voice, pursuant to my employment.

My signature will acknowledge that no implied promises, obligations or guarantees in connection with this application have been made to me by Old Forge Properties.

Hereby authorize Old Forge Properties to seek medical treatment for injuries or illness sustained by employee during the course of his/her employment with us. Old Forge Properties will make a conscientious effort to notify parents or guardians should treatment become necessary for the dependent child.

Do hereby release and hold harmless Old Forge Properties from any and all claims whatsoever, including but not limited to personal injury, arising out of or relating to any non-work hour and/or non-work related recreational activity provided to employee by or on behalf of Old Forge Properties.
